Quick Investigation Kit
Investigating slips, trips, falls and minor errors that do not result in lost-time injuries takes up a disproportionate amount of management time, but they are important because knowing why they happened allows action to be taken that prevents more serious events.
To deal with such minor events, we have just introduced the Quick Investigation Kit.
The pack contains:
- One A3 laminated chart for use with wipe-off markers
- One A3 25-leaf, paper chart pad
- An A3 detailed instructions document, including sample reports
- A TOP-SET® pen
- A pack of three mini Post-its™
- A wipe-off marker for use with the laminated chart
All presented in a strong, black polyethylene case.
The charts take you right through the TOP-SET® process, from providing an initial incident statement to developing a root cause analysis and recommendations. Using nine easy to follow steps, you conduct your investigation by entering information into preprinted boxes on the charts. There is a complete worked example on the reverse of each A3 chart.
We have piloted this system in several countries and proved to our satisfaction that small incidents can be investigated and reported on in as little as 20 minutes - the Quick Investigation Kit is an effective tool and real time and cost saver.
