Investigators' Reporting Workbook
The Investigators' Reporting Workbook is an A4-sized, bound notebook designed to help store information during the planning and information gathering stages of an investigation. It allows investigators to keep all of their incident investigation data in one place.
The information can then be used to create a Storyboard and Root Cause Analysis, and to help create a final report.
Information is simply entered into standardised forms: for example, the terms of reference, the initial incident report, sequence of events, evidence log, interviews, and recommendations. A suggested final report format is also included.
The TOP-SET indicators (part of the TOP-SET methodology) are laid out to help users to plan the investigation, and set about investigating and gathering information.
