Learn to use the TOP-SET system to enhance your investigation skills and become an active member of your incident investigation team. Course objectives.
The 1-Day Investigator course is specifically designed for managers, supervisors and safety representatives who are required to take part in incident investigations but not necessarily required to lead the investigation team. The course introduces delegates to the Kelvin TOP-SET incident investigation system and teaches participants the specific steps of investigating an incident using the TOP-SET process. A variety of short case studies and worked examples are used to stimulate discussion and reinforce the concepts being taught.
Some of the concepts explored during the course include the significance of change, and how missing barriers and defences and the by-passing of controls can influence the circumstances leading to an incident. The course also pays particular attention to the investigation of near hits and minor incidents as a method of preventing more serious incidents from occurring. The learning objectives are similar to that of the Kelvin TOP-SET 3-Day Senior Investigator Course but at a much more basic level.
After attending this course delegates will be able to actively participate in an incident investigation team and will have a good understanding of how to investigate an incident using the TOP-SET methodology.
This course is run on an open basis in the UK and internationally and can also be run as an in-house course on request.

Participants at the end of the course ought to:
· Understand the TOP-SET process
o Participants are given an overview of the TOP-SET incident investigation system
· Appreciate the value of the Action Card
o The Action Card is at the heart of TOP-SET. It is designed to help the investigator follow the system and stay on track
· Be aware of the importance of the Incident Statement
o The Incident Statement is the key to the investigation and the subsequent Root Cause Analysis
· Know how to plan an investigation
o Planning has many aspects including the scope and boundaries which may limit your influence
· Recognise the value of working as a team
o The varied contribution of team members and their productive co-operation is emphasised throughout
· Be able use Indicators to collect data
o The TOP-SET system promotes wide and open thinking using the Indicator Card
· Know how to record information on the Storyboard
o The Storyboard is the repository of all information relating to the incident and relies on the effectiveness and flexibility of Post-its
· Be able to construct a Root Cause Analysis
o The development of a logical analysis is the culmination of a successful investigation
· Appreciate the importance of SMART actions
o Effective SMART actions are the goal of any worthwhile investigation
Participants will receive a Kelvin TOP-SET ‘Investigator’ certificate on successful completion of the course.
